Failed to conduct monthly and annual inspection per Section 3.7.2 and 3.7.3, respectively, of the <br /> SPCC Plan, and inspection report form as provided on Appendix B of the SPCC Plan were not found on <br /> site. Conduct inspections and tests required by this part in accordance with written procedures that you <br /> or the certifying engineer develop for the facility. You must keep these written procedures and a record <br /> of the inspections and tests, signed by the appropriate supervisor or inspector, with the SPCC Plan for a <br /> period of three years. Records of inspections and tests kept under usual and customary business <br /> practices will suffice for purposes of this paragraph. Immediately begin conducting inspection in <br /> accordance to written procedures developed by you or the certifying engineer, and retain a copy of the <br /> appropriately signed report of the inspection with the SPCC Plan for minimum of three years available <br /> for inspection. Submit last three years (if available) of monthly and annual inspection report to the EHD <br /> by August 26, 2013. <br /> 518. Aboveground petroleum storage training is not being conducted per Section 3.8.2, 3.8.3 of the <br /> SPCC Plan, and records of the training/briefing per Section 3.8.4 were not found on site. At a minimum, <br /> oil-handling personnel must be trained in the operation and maintenance of equipment to prevent <br /> discharges, discharge procedure protocols, applicable pollution control laws, rules & regulations, general <br /> facility operations, and the contents of the facility's SPCC Plan. Annual briefings must be conducted to <br /> assure adequate understanding of the SPCC Plan and highlight and describe any known discharges or <br /> failures, malfunctioning components, and any recently developed precautionary measures. Immediately <br /> conduct necessary training for all oil handling personnel.
