0 0 <br /> 1.0 EMERGENCY CONTACT PROCEDURES <br /> 1. In the event of an incident, the following outside resources can be <br /> contacted by the Incident Commander (IC) or designee for immediate <br /> assistance (see Emergency Contact List at the end of this Appendix for <br /> telephone numbers). <br /> 2. Fire Department: when conditions are such that a fire, explosion or <br /> release of hazardous materials may result. <br /> 3. Police Department: when conditions are such that traffic control may <br /> become a problem, or evacuation is necessary, or the safety and security <br /> of employees is at risk. <br /> 4. Ambulance/Hospital: in the event of serious injuries to personnel. <br /> 5. Spill Contractor: when a spill cannot be controlled or cleaned up with the <br /> available materials at the site, or when the Incident Commander deems <br /> that a professional spill response firm may be more effective. <br /> 6. State and Federal Agencies: When a spill of hazardous material or oil <br /> occurs. <br /> 7. Local Emergency Planning Committee: when assistance is needed in the <br /> coordination of an evacuation or clean-up. <br /> 8. The following information is to be provided to the outside resource <br /> regarding the incident: <br /> • Facility name, address, location, and telephone number; <br /> • Name of person reporting the incident; <br /> + Date and time of incident; <br /> • Material that was released; <br /> • Source of material that was released; <br /> • Quantity of material that was released; <br /> • Media impacted by release (air, soil, and groundwater); <br /> • Status of incident(ongoing or completed); <br /> • Initial remedial measures undertaken (containment, none); and <br /> • Injuries associated with the incident. <br /> See Appendix 3 for detailed reporting guidelines. <br /> Pacific Ethanol Stockton, LLC October 2008 <br /> Integrated Contingency Plan Page: 1-1 <br />
