SPCC Plan lacks full approval of management at a level of authority to commit the necessary <br /> resources to implement the Plan. The Plan listed an outdated personnel, Director of Environmental <br /> Operations UPRR, to provide management approval. Correct by adding an addendum to the SPCC Plan <br /> that lists the current personnel who has the authority to provide resources to implement the Plan. <br /> # 18. Facility diagram lacks location of connecting pipe lines. Current facility diagrams shows location of <br /> AST tanks and oil storage room but lacks the connecting pipe line from the used oil tank to the used oil <br /> pump located in the crane pad. Diagram does not include pipe line from delivery pipe line to AST and to <br /> the dispensers. Facility diagram should include basic connecting pipes and intra-facility gathering lines. <br /> Correct by revising the facility diagram to show the pipe lines layouts. <br /> # 24. SPCC Plan failed to have current phone numbers of local agencies. The contact phone number <br /> lists an outdated phone number for San Joaquin County Office of Emergency Services. Contact list and <br /> phone numbers should be current for the facility response coordinator, National Response Center, <br /> cleanup contractors whom you have an agreement for response, all appropriate Federal, State and local <br /> agencies who must be contacted in case of discharge. <br /> # 48. Failed to keep records of inspections on tanks, containers and supports. An facility internal <br /> inspection report stated that the Fuel Pad was inspected. There was no description of what was <br /> thoroughly inspected. In the SPCC Plan, it stated a visual daily inspection will be on the tanks and an <br /> annual inspection on the tank structure and piping. No records were available for review. Correct by <br /> either testing/ inspecting each aboveground container for integrity on a regular schedule and whenever <br /> you make material repairs. Retain comparison records and inspect container's support and foundations. <br /> Frequently inspecting the outside of container for signs of deterioration, discharges, or accumulation of <br /> oil inside the detection ponds. Records must be retained onsite for a minimum of 3 years. Submit a <br /> statement how this will be conducted. <br /> Submit to SJC EHD within 30 days (10/25/10): <br /> 1. a copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for the above violations <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> 464 <br /> EHD 23-n-003 Re /10/10 CONTINUATION FORM <br />
