Y .y <br /> St. Joseph's Medical Center <br /> 2013 CUPA Inspection Return to Compliance <br /> Above Ground Storage Tank Program Inspection Report Nov. 5, 2013 <br /> Violation: Item#504, 505,CFR 112.7(a) (3)(i) <br /> No Facility diagram or diagram failed to show location and contents of each container,transfer stations <br /> and connecting pipes. <br /> Correction: The spcc plan has been amended to include the additional oil filled containers.The plan has <br /> also been recertified by a professional engineer. <br /> Violation: Item#517 CFR 112.7(e) <br /> The facility failed to document inspections as specified in the SPCC plan. <br /> Correction:Although the generators are inspected on a daily basis by the watch engineer,the monthly <br /> inspection was conducted on November 20, 2013 on all generators and tanks during the EPSS tests.This <br /> document will reside in the AST Logbook. Future inspections will be carried out and documented in the <br /> same manner. <br /> Violation: Item#518,CFR 112.7(f) <br /> The facility failed to document employee training as specified in the SPCC plan. <br /> Correction: In December of 2013 at our regularly scheduled crew meeting, spill training was given <br /> according to the guidelines stated in the SPCC plan.The training was documented and placed in the AST <br /> Logbook.The annual training is now set for each December going forward. <br /> Violation: Item#607,CFR 112.8(c)(2) <br /> Facility personnel could not demonstrate that the following diesel containers had secondary <br /> containment, 109C, 109HCCL, 109P. <br /> Correction: Additional Information on the existing diesel tanks for the aforementioned equipment has <br /> been supplied by Holt Of California and given to San Joaquin County Environmental Health Department. <br /> All of the tanks in question are double walled. <br />
