A tracking spreadsheet that presents the <br />violations, action items, and final status of each of the action items is presented in Attachment 2. <br />To summarize the action items, a facility -wide Spill Prevention, Control and Countermeasure <br />Plan was created that addresses the findings. The plan went into effect May 4, 2016.
