Driving personnel were last <br />trained on 9-4-14 and shop personnel were last trained on 9-5-14. Personnel who handle hazardous waste shall be <br />trained within six months of assignment and take part in an annual review of the initial training received. At a <br />minimum, the training program shall be designed to ensure that facility personnel are able to respond effectively to <br />emergencies by familiarizing them with emergency procedures, emergency equipment, and emergency systems, <br />including where applicable: <br />1. procedures for using, inspecting, repairing, and replacing facility emergency and monitoring equipment; <br />2. key parameters for automatic waste feed cut-off systems; <br />3. communications or alarm systems; <br />4. response to fires or explosions; <br />5. response to ground -water contamination incidents; and <br />6. shutdown of operations. <br />The owner or operator shall maintain the following documents and records at the facility: <br />1. the job title for each position at the facility related to hazardous waste management, and the name of each <br />employee filling each job; <br />2. written job description for each position listed above; <br />3. written description of the type and amount of both introductory and continuing training that will be given to each <br />person filing positions listed above, <br />4. records that document that the training has been given to and completed by facility personnel. <br />Training records on current employees shall be kept until closure of the facility. Training records on former employees <br />shall be kept for at least three years from the date the employee last worked at the facility. Immediately provide <br />employees with initial or refresher hazardous waste training. Submit a copy of the roster and the syllabus to the EHD. <br />This is a Class II violation. <br />301 <br />CCR 66265.31 Facility not maintained to minimize the release of a hazardous waste. <br />Dark oily stains were found on the cement outside the maintenance shop south of the bay doors, on the cement <br />outside next to the five -hundred gallon hydraulic oil tank number 5 and five -hundred gallon semi -synthetic diesel engine <br />oil tank number 6, and outside at the hazardous waste storage area. Broken fluorescent tube pieces and metal fines <br />were left on the secondary maintenance shop floor. Facilities shall be maintained and operated to minimize the <br />possibility of a fire, explosion or release of hazardous waste to air, soil, or surface water which could threaten human <br />health or the environment. Immediately clean up the dark oily stains outside the maintenance shop, the area near the <br />oil tanks, and the hazardous waste storage area, and clean up the broken fluorescent tube pieces and metal fines <br />from the secondary maintenance shop floor, and manage according to Title 22 hazardous waste regulations.
