REI -IS <br />x— Z4 1z <br />SR0059375 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the AMENDED application for a permit to install Veeder Root 8484 PLLDs, Bravo B-401 tank <br />top sumps and collars, Bravo F -17 -RR -D penetration fittings, Fiberglass Systems penetration <br />fittings, Fiberglass Systems Red Thread IIA pipe and Bravo under dispenser containments <br />(UDCs) (B8000) on the underground storage tank (UST) system at the above noted address. <br />The permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Garrett Backus at (209) 468-2986 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). <br />a. Soil sampling under the UDCs being removed. <br />b. Primary product piping soap test: One hour pneumatic pressure test at 110% of <br />operating pressure, or 40 psi, whichever is greater. <br />c. Primary vent and vapor piping soap test: One hour pneumatic pressure test at 110% <br />of operating pressure, or 5 psi, whichever is greater. <br />d. Secondary containment (UDCs, tank top sumps and vent sump) soap test: One hour <br />pressure test of the sump interstitial space per sump manufacturer's guidelines. If <br />there are no manufacturer's guidelines, test shall be performed using an applicable <br />method specified in an industry code or engineering standard. <br />e. Secondary product, vent and vapor piping soap test: One hour pressure test per <br />manufacturer's guidelines. If there are no manufacturer's guidelines, test shall be <br />performed using an applicable method specified in an industry code or engineering <br />standard. <br />f. Leak detector test: Leak detectors must be tested to confirm that the devices can <br />detect a leak of 3 gallons per hour at 10 psi. <br />g. Spill bucket test: Pass an one hour hydrostatic test (or equivalent) for the spill <br />buckets dismantled and reassembled during construction. <br />
