Please <br />complete the following items within 30 days of the close of escrow and return to the EHD <br />(Health and Safety Code section 25284(b -c). <br />1. Complete the enclosed Service Request form and arrange payment of the transfer fee <br />and the one hour UST consulting fee. <br />2. Contact Garrett Backus at (209) 468-2986 to schedule a new owner consultation <br />meeting once escrow has closed and obtain all needed forms. <br />3. Underground Storage Tanks — Facility Form — Provide all current information, sign <br />and date at the bottom. <br />4. Underground Storage Tanks — Tank Forms — Provide all current information, sign <br />and date the bottom of each form. Complete one form for each tank or compartment <br />of a split tank. <br />5. Certification of Financial Responsibility and Financial Responsibility Mechanism. <br />These documents are required to be submitted to this office annually. <br />6. Provide documentation (Notice of Sale or Grant Deed) indicating when escrow <br />closed and name(s) of previous and new owner(s). <br />7. Underground Storage Tank Monitoring and Response plans. The EHD will review <br />and return approved copies to the tank owner/operator. <br />8. Report #5021: Update all information for all programs at this site(s) including the <br />UST and Hazardous Waste programs, sign and date. <br />9. Complete and return the form "Owner Statements of Designated Underground <br />Storage Tank (UST) Operator and Understanding of and Compliance with UST <br />Requirements." <br />10. Go to: http://www.waterboards.ca.gov/water—issues/programs/ust to obtain the <br />current statutes (Underground Storage of Hazardous Substances) and UST <br />Regulations that you are required to have for your monitoring and response plans. <br />Page 1 of 2 <br />
