SAN 10 A 4 U I N Environmental Health Department <br />r`_0U;1J.I.,I.__ <br />Complaint Record Daily Time and Activity Summary from 8/22/2013 to <br />8/22/2018 for C00047296 located at <br />4614 W TURNER RD <br />Activity Date Recorded by <br />08/21/2018 Received page from front desk. Yvonne received a call from Tom Tapia that there PE 2546 BAKER <br />30 min was an ammonia leak at 4614 W. Turner Road in Lodi. There were no exposures SC 007 <br />or evacuations. Contact numbers are 209-915-0068 & 209-334-8989. <br />Discussed complaint with Garrett. Called Tom Tapia at 209-915-0068 and left a <br />message. Called 209-334-8989 and spoke with Tom Tapia. He confirmed the <br />information in the complaint. He said that they determined the release to be less <br />than a pound of ammonia and the leak has been fixed. The ammonia was <br />released to the air only. He said that the leak came from the ammonia valve <br />strainer assembly going into the wine tank. One of the gaskets had started to <br />deteriorate. <br />Passed information to Haza so he could follow-up. <br />08/21/2018 Lydia Baker with EHD discussed the Complaint with me. I called the Site PE 2546 SAEED <br />12 min maintenance manager, Gabino Pelayo, and discussed the complaint with him. No SC 007 <br />Exposure/Injuries/Evacuation. <br />An Employee smelled an ammonia during the walkaround, and notified <br />maintenance. Maintenance were able to trace the leak to a strainer gasket on the <br />one of the bottom pipes. They shut off the valve and stopped the leak. Gasket is <br />being replaced today. <br />Narratives were entered with PR0535200 for any follow ups. <br />08/22/2018 Abated complaint as office response only and gave to Garrett for review.
