The MLLD <br />was installed on November 13, 2007 and passed the three gallon per hour at ten psi <br />functionality test. The permit has been approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution <br />Control District for any applicable permits and requirements. <br />2. A "Certificate of Compliance for Underground Storage Tank Installation <br />Form C" shall be submitted by the owner or the owners' agent [pursuant to <br />(CCR) 2635(d)] before this repair will be considered complete by the EHD. <br />3. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all <br />of the following: <br />a. One of several permissible valid, current contractor's licenses issued by <br />the Contractors State License Board (may be issued to the installation <br />company), and <br />b. A valid, current California UST Service Technician certificate issued by <br />the International Code Council (ICC), and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced, and <br />d. Each person installing or testing any component must hold a certificate of <br />training from the manufacturer of that component. <br />4. Permit expires 90 days from date of approval. Note: If an extension to the <br />expiration date is required, a written request must be submitted to this office prior <br />to the expiration date. <br />
