LEAK a K TUNTERCEPTTON AND DETECTION SYS'T'EM _
I i.; information is required t ndy for motor vehicle fuel UST systems constructed per the Alternate Constntcfion Requirements of 23 CCR §2633, and only rf the Leek
Interception turd Iktection System (LIDS) dM-nol meet tete volumetric requirements of 23 CCR §2631(d)(1) through (3) (i.m. when accounting for rainfall and
backfill material, the secondary cotisinment votume is less than 100% of primary lank volume for a single UST; or in the case of tn8ndliple USTs in shared secondary
containment, 130% of the largest primary tank volume or 10% of aggregate primary tank volume, whichever is greater).
AT l'ACH AN ADDITIONAL FACE TO THIS PLAN CONTAINING THE FOLLOWING INFORMATION:
The vohnne, orthe LIDS in relation to the volume of the primary container;
> TMc amount of time the LIDS shall provide conte r.mcnt related to the ;me between detection of an unauthorized rt lease Lind cleanup of the leaked subsiarme:
9 The depth from the bnttnm of the IJDS Will! highest anticipalod level of groundwater:
> The nature of the unsaturated soils under the LIDS and their ability to absorb contaminants or to allow movement of contaminants;
P The methods and scheduling for removal of all hazardous substances which may have been discharged from phuTnary contau,mew and Hie loalcd ht ;he
unsaturated soilsbrmeerl the priumry contam^.tatt and gioundwalor, including !he LIDS sump.
`iI. REPORTING AND RECORD KEEPING
We will reporUrecord any overfill, spill, or unauthorized release from a UST system as indicated in this plan.
Recordable Releases: Anv unauthorized rehxse from primary umtsinmtnt which the UST operator is able to clean up within eight (8) hours after the release was
detected or should reasonably have been detected, and which does not escape from secondary confiimttmtt, does not bacrene the hazard of fire or explosion, kind does
not cause any deterioration of secondary comainment, must be recorded in the fucilito rnnaitoring records. Monitoring records must includes
> The UST oporslor'a name and telephone number,
> A list of the types, quantities, and concentrations of hazardous substances released;
> A description of the actions taken to control and clean up the roloase;
D The, method and location of disposed of the released hazardous substances, and whether a hazardous waste manifest was or will be used;
Y A description of actions taker, to repair the UST and to prevent future releases;
> A dcomp±ion of the method used to reactivate interstitial monitoring after replacement or repair of pdmary containment.
Reportable Releases: Any overfill. spill, or umauthmized release which escapes from secondary containment (or primary containment if no secondary containmeni
exists), increases tine hazard of fire err explosion, or causes arty deterioration of seconder coatainmmrt. is a reportable release. titpertabic releases arc also rmordable.
Within 24 hours otter a reportable retoase has been detected, or should have been detected, we will notify the local agency administering the UST program of etc
release, invu:% igale the release, and take Immediate messtrrea to stop tem reles te. IFtmrasnry, or if required by tete beat agency, remaining stored productiwaste will
he removed from the UST in prevent further releases or filcilitate corrective action if as ernergency Lutists, wo will noli6- she Steno 0rce of Emergency Services.
Within five (5) working days of a reportable release, we will submit to the local agency a full written report containing all of the following information to the extent
That the information is known at the lima of tiling the repurt;
➢ The UST owners or operstot't mese and tekphone myrib r;
➢ A list of the types, quantities, and concentrations of hazardous minetrittls released:
D The approximate date of the release;
> The date on which tho release was discovered;
D 'V he date on which the micasc was stoppai;
4 A descrption of actio:.s taken to control and/or slap the release;
> A description of corrective and remedial actions, inchtding investigations whieb were undertaken and will be conducted to determine the nature and extent of soil,
ground water or surface water contamination due to the release;
➢ The metfioci(v,, of elatrup inepicavoited to date, propaaod cleanup actions, mid a aelied .le for implementing the propose] actions;
> The method(s) and locations) of dispmad of relaxed 6azrtrdous materials and any contaminated soils, groundwater, or surface water,
D Copies of any hazardous waste manifests used for off side transport ofhazardous wastes associated with clean-up activity;
> A description of proposed methods for any repair or rcplscatneni of UST system primary/secondary containmatt systems;
> A description of additional actions taken to prevent future rctcxsra.
We will follow the re pcirtinR procedures describod above if any of the following conditimt9 occur:
D A recordable unauthorized release can not becdeaned up or is still under investigation within eight (8) horns of detection;
> Rateased hazardous substance are discovered at the UST site or in the surrounding arra;
D Unusual operating conditions are observed, inctuding cratic behavior of product diapcnaing copipment, sudden loss of product, or the unexplained presence of
water in the tank, unless system equipment is fotmd to be defective and is immediatrJy repaired or replaced, and no leak huts occurred;
D Monitoring results [mm UST system monitoring equipmenlintetbndt Indicate that a release may have occurred, unless the monitoring equipment is found to be
defective and is immediately repaired, recalibrated, or tsphced, and addititmai tiiouitoring does not confirm the initial rssulm
Record detention: Monitining records and written reports of unauthorized releases must be maintained on-aits (or off-site at a readily available location, if approved
by the local agency) for at least 3 years. Hazardous waste shipping/disposal records (a.d, manifests) must be maintained for at least 3 years from the date of stuptile" t
CERTIFICA
VVI, OWNERIOPERATOR SIGNATURE
of the Information provided herein U truo and accurate to the best of
-/40-0)6
OWN R10PE3tA AMl3(prhtt) 8x1 i OWNEiR/OFF.RATORTiTLE xrt.
Bat ire— ,C V , Manager, - + >
1 (Agertev lire tiny) —il.istp`lan has been reviewed and )(Approved !1 A"roved With Conditions ❑ Disapproved
' Local Agency Signature:.yV • N Date: I ( ~IL' -0 (a - -
hrvftvrc-e (M e34) -V4
