The following items must be submitted <br />and deficiencies/discrepancies must be corrected prior to approval: <br />1. The installation permit requires a signature of the responsible party to be billed in the <br />event that additional EHD staff time is expended. A signature of a Jack In The Box <br />representative on this application must be submitted, if Jack In The Box is indeed the <br />responsible party to be billed. A signature of Don Esperson of Western States <br />Contracting & Engineering is on the application. Submit a copy of this page with the <br />correct billing information and signatures. <br />2. The dispenser pans and sumps are not third party tested. It is required that third -party <br />tested components be used. Submit manufacturer specification sheets for the equipment <br />to be installed. <br />3. Submit copies of the contractor certification for the equipment to be installed including the <br />monitoring equipment. <br />4. Submit copies of the International Code Council UST Installer/Retrofitter certification. At <br />least one person who holds this certification must be on the jobsite while work is being <br />performed. <br />5. Submit copies of the International Code Council California UST Service Technician <br />certification. At least one person who holds this certification must be on the jobsite while <br />work is being performed on the monitoring equipment. <br />6. Submit copies of the buoyancy calculations. <br />7. There is a discrepancy with what type of annular sensor is to be used. Both single float <br />and dual float are named in the permit application and plans. Specify which is to be used. <br />8. Plans do not specify the vacuum source. Specify vacuum source. <br />9. Specify where vapor vacuum monitoring will be located. There is a discrepancy in the <br />plan showing they are located in both 87 & 91 fill sumps. <br />10. Specify what method of sump monitoring is to be used. Plans make it seem that either <br />vacuum or hydrostatic may be used. <br />11. Plans show there is vapor recovery for the diesel. Is this correct? <br />12. Healy System — will it be capped or will it lead back into the system <br />Please remit the requested information and plan updates to my attention.
