Attached is a letter <br />to the U.S. EPA explaining what happened with the State <br />and Federal numbers. I also attached a copy of the 1990 <br />correspondence from the State which issued me the State <br />I.D. # CAL 000 039 919 to replace the EPA I.D. # <br />CAD 099 118 910, as I explained when I checked with the <br />U.S. EPA and with your office. Everyone agrees that I <br />should not have been issued the State #• <br />By this letter, I am requesting that you deactivate <br />the state I.D. # CAL 000 039 919 and show us as a large <br />generator using our original Federal I.D. #• The origina} <br />Federal I.D. # which is still active in the U.S. EPA <br />computer is CAD 009 118 910. Note that this # was one <br />number off from the number we had been using prior to the <br />State number being issued. <br />Because of these number problems, I was not mailed a <br />Biennial Report request for 1991. I did get the Report <br />Forms from your office recently after I requested them.
