Piccoli Road, Stockton Program: APSA <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II, or MINOR-Notice to Comply) <br /> Routine Aboveground Petroleum Storage Act Inspection report continue <br /> # 32. Failed to (1) train personnel on discharge prevention and (3) schedule discharge prevention <br /> briefings annually and (2) designate a person to be accountable for discharge prevention. The Plan <br /> stated that training records were kept with the Plan for a minimum of 3 years. No records were found. <br /> Train your oil-handling personnel in operation and maintenance of equipment to prevent discharges; <br /> discharge procedure protocols; applicable pollution control laws, rules and regulations; facility operations <br /> and the contents of SPCC. Briefings should be held annually. Designate a person who is accountable for <br /> discharge prevention and who reports to upper facility management. Correct by revising the Plan to <br /> include the above information. <br /> # 33. Failed to describe how oil handling, processing and storage area are secured and controlled; how <br /> unauthorized access to starter controls on oil pumps are prevented; and the appropriateness of security <br /> lighting. Description of the above information not found in Plan. For example, an incident had occurred in <br /> April 2010, when the oily-waste water steam cleaner tank had overflow into storm drained due to pump <br /> being left on. Describe in the Plan how you secure and control access to oil handling, processing and <br /> storage areas: secure master flow and drain valves of waste that may flow to leach fields and drainage <br /> ponds; prevent unauthorized access to starter controls on oil pumps; and the appropriateness of <br /> security lighting to both prevent acts of vandalism and assist in the discovery of oil discharges. <br /> Submit to SJC EHD within 30 days (01-03-2011): <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for each of the above violations <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD In p ctor: ived By: Title: <br /> EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
