Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />April 12, 2018 <br /> Facility Address: <br /> 700 E ROTH RD, LATHROP <br /> Facility Name: <br /> AAFES WEST COAST DISTRIB CENTER <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 4 HSC 25508(a)(1) Failed to complete and/or submit the Business Activities or Owner/Operator Identification page. <br />The Business Owner/Operator Identification form was not submitted for 2018. This information must be current <br />and submitted as part of the business plan. Immediately log into the California Environmental Reporting System <br />(CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 5 HSC 25506(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br />Building 590 has 55 gallons of new antifreeze, 110 gallons of grease, and 165 gallons of wind shield wiper fluid were <br />not reported on inventory. The fuel island has 12,000 gallons of diesel and 500 gallons of diesel exhaust fluid that <br />were not reported on inventory. The fire pump house has 500 gallons of diesel that was not reported on inventory. <br />The generator has 350 gallons of diesel that was not reported on inventory. The shop area has 282 cubic feet of <br />oxygen that is not being reported on inventory. Building 550 has 110 gallons of used oil, 55 gallons of used <br />antifreeze, 275 gallons of hydraulic fluid that s not being reported on inventory. Any material that meets or exceeds <br />the reportable quantity shall be reported in the facility's business plan. Immediately log into the California <br />Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material <br />inventory, and submit to the EHD for approval. <br />This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br />A site map was not complete as part of the business plan. The site map is missing a loading area, storm drain <br />locations, sewer drain locations, access locations, exit locations, emergency response equipment locations, <br />emergency shut offs, and an evacuation staging area. A site map shall contain a north orientation, loading areas, <br />internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation <br />staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map <br />element is not applicable for your facility then list it on the map and label as “NA". Immediately log into the California <br />Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, <br />and submit to the EHD for approval. <br />This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br />release. <br />The emergency response procedures were not submitted for 2018.
