The site map did not contain the location of the <br />loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, <br />evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. <br />A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br />access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br />areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br />map and label as "NA". Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br />7 <br />HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br />release. <br />The emergency response procedures were not completed with all the required information The emergency <br />contingency/response plan submitted to CERS did not include the location of emergency response equipment. <br />The facility listed the CUPA Fax number instead of the phone number for notification in case of an emergency. A <br />business plan shall include the following emergency response procedures for a release or threatened release of <br />hazardous materials, including, but not limited to, the following: <br />- immediate notification of local emergency personnel and the Environmental Health Department (EHD): <br />(209)468-3420 <br />- procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br />- evacuation plans and procedures, including immediate notice, for the site <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br />correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Onsite to Conduct a Hazardous Materials Business Plan (HMBP) Inspection. There were two violations during <br />the inspection. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a <br />statement documenting the corrective actions that have been or will be taken for each violation, and any <br />supporting paperwork, by 10/18/2018 . <br />Starting September 1, 2018, all in -office CERS help will be provided at EHD hourly rate ($152). To schedule an <br />appointment, please call (209) 468-3420. <br />Notes: <br />Facility Listed several chemicals in CERS chemical inventory under the threshold reportable quantities. The <br />facility is only required to report at any one time to report any hazardous materials at or above 55 gallons for <br />liquids, 500 pounds for solids, or 200 cubic feet for compressed gases (include liquids in ASTs and USTs); or is <br />regulated under more restrictive local inventory reporting requirements (shown below if present); or the <br />applicable Federal threshold quantity for an extremely hazardous substance specified in 40 CFR Part 355, <br />Appendix A or B; or handle radiological materials in quantities for which an emergency plan is required pursuant <br />FA0024331 PR0543440 SCO01 09/18/2018 <br />EHD 19-01 Rev. 09/22/16 Page 2 of 3 Hazardous Materials Program OIR <br />
