Date: 07/06/2019 <br />Manufacturer training: VMI, 07/12/19; Veeder Root, 06/30/19; Bravo, 09/11/19 <br />OVERALL INSPECTION NOTES AND COMMENTS <br />NOTES <br />On site to verify dispenser foot prints contained under dispenser containment sumps, brine leaks in interstitial space of all <br />sumps and UDC's, communication for all VPH components , leak detector test, monitoring system functionality and final <br />inspection and demonstration of automatic shutdown. All observed tests passed. <br />Facility is not authorized to operate until all violations associated with the 10/18/2017 monitoring system certification report, or <br />any amended versions of that report, are addressed and all requirements for service request SR0076200 are met. Among the <br />requirements that need to be met are the submission of the California Environmental Reporting System (CERS) and all <br />applicable hazardous materials, hazardous waste and UST components, an accurate “Certification of Installation / <br />Modification” form for each tank shall be submitted into CERS and, an accurate and drawn to scale as-built plan must be <br />sumitted to the EHD. All submittals must be reviewed and approved bythe EHD. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
