These records shall <br />be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or <br />sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) <br />results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring <br />equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. <br />Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and <br />submit copies to the EHD. <br />This is a minor violation. <br />207 <br />CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator failed to check and note the correct test dates for the secondary containment test on the <br />October 2014 to July 2015 designated operator monthly inspection reports. The reports documented that the <br />secondary containment was completed on 9/27/10 when secondary containment testing was completed on 9/24/13. <br />During the monthly inspection, the designated operator shall check that all required testing and maintenance for the <br />UST system have been completed and document the dates they were done. Ensure that designated operators <br />performing monthly inspections at this facility are including all of the required information on the reports. <br />The designated operator failed to document all the alarms from the attached alarm history on the December 2014 <br />designated operator monthly inspection report and failed to check that they were responded to appropriately. The <br />missing alarms include: L3 under dispenser containment sensor fuel alarms on December 2 and 11, 2014. During the <br />monthly inspection, the designated operator shall review the alarm history for the previous month, check that each <br />alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation <br />taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly <br />inspections at this facility are including all of the required information on the reports. <br />This is a repeat violation, Class II. <br />319 <br />HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre -Jul 2003). <br />Approximately one inch of liquid was observed in the under dispenser containment sump. If water could enter into the <br />secondary containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately <br />remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it <br />accordingly.
