Hazelton Ave., Stockton, CA 95205-6232 To whom it may concern, 10/21/2014 NOV 0 3 2014 ENVIRONMENTAL HEALTH DEPARTMENT On October 6, 2014 we were visited by the San Joaquin County Environmental Health Department for the annual inspection of our underground storage tank. There were some violations that were discovered at the time of the inspection. This note is to signify that I have corrected each of the violations and will address how they will be prevented in the future. 103. Current Financial Responsibility Documents. Facility manager will acquire a copy of the current liability documents from the Toys R Us corporate office on or around July 1 of each year and place a copy of the document in the compliance binder. A copy will also be sent with the Financial Responsibility document to the Environmental Health Department on or before October 1 of each year. In addition, EHD Certification form has been submitted to CERS. 107. Accurate UST Monitoring Site Plan was not submitted to CERS. This has been submitted and accepted on 10/20/2014. 115. An Owner Statement of Designated UST Operator Compliance was not submitted to CERS. There were no changes; however the form was updated with the DO license date and uploaded to CERS. 207. The Designated Operator Failed to check and note correct test dates for the Triennial secondary containment testing on the DO monthly inspection reports. These reports have been updated by the DO. Designated operator failed to verify all employees have been trained during Nov.2013, Mar 2014, Apr 2014, May 2014, June 2014, Aug. 2014. This has been corrected and Owner will verify monthly.
