Hazelton Ave., <br />Stockton, CA 95205-6232 <br />To whom it may concern, <br />On October 6, 2014 we were visited by the San Joaquin County Environmental Health Department for <br />the annual inspection of our underground storage tank. There were some violations that were <br />discovered at the time of the inspection. This note is to signify that I have corrected each of the <br />violations and will address how they will be prevented in the future. <br />103. Current Financial Responsibility Documents. Facility manager will acquire a copy of the current <br />liability documents from the Toys R Us corporate office on or around July 1 of each year and place a <br />copy of the document in the compliance binder. A copy will also be sent with the Financial Responsibility <br />document to the Environmental Health Department on or before October 1 of each year. In addition, <br />EHD Certification form will be submitted. <br />107. Accurate UST Monitoring Site Plan was not submitted to CERS. This has been submitted and <br />accepted on 10/20/2014„ F <br />115. An Owner Statement of Designated UST'Operator Compliance was not submitted to CERS. There <br />were no changes; however the form was updated with the DO license date. <br />207. The Designated Operator Failed to check and note correct test dates for the Triennial secondary <br />containment testing on the DO monthly inspection reports. These reports have been updated by the DO. <br />Designated operator failed to verify all employees have been trained during Nov.2013, Mar 2014, Apr <br />2014, May 2014, June 2014, Aug. 2014.
