The listed factors need to be corrected before the permit can be <br />approved: <br />1. State contractor information requested on page 1 of application <br />2. Applicant signature is needed on page 1 of appication. <br />3. State sampling firm and analytical laboratory. <br />4. State what will be done with excavated soil. <br />5. Clarify method of decontamination. <br />6. Clarify how rinsate will be stored and/or transported off site. <br />7. Submit manufactures training certification for all equipment to be installed. <br />8.
