This information has not been <br />received resulting in a non-compliant status for this facility. <br />Failure to correct the violations will result in continuing/additional violations. <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is <br />billed at the current hourly rate ($125).
