The permit las been <br />approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2- Any changes in information and scope of work must be submitted to EHD for approval <br />prior m commencing work. All new addenda supersede the previous ones. <br />3- Contact Stacy Rivera at (209) 4683440 to arrange the fofowing inspection <br />appointments (a 48-hour notlee Is required for Inspection appointments). Please be <br />aware that inspections for emergency work must be scheduled within five days W pelmi <br />approval. <br />a. Visual inspection of diesel piping within dispenser <br />b. Secondary containment testing on UDC 213 <br />4. If concrsie will <br />be broken near the UST or piping, a secondary containment test must be <br />performed on the UST or piping ones concrete is poured in accordance with the <br />manufacturers guidelines. <br />5. Any individual performing Ilhe work of a service lachnidan must meet the following <br />qualifications: <br />a. Possess a -be employed by a person who possesses one of several permissible <br />valid, current contractor's licenses issued by the Contractors Stale License Board <br />(may be Issued to the Installation company), or a Tank Tester's license Issued to the <br />individual by the Stale Water Board, and <br />b. Possess or work urder the direct and personal supervision of an individual physically <br />present at the work site who possesses a valid, current California LIST Service <br />lechrrcian cer:rficate issued by the International Code Council (ICC), and <br />
