The test boots in the diesel tank were not pulled back. The opening in the boots was <br />observed facing up. Any potential release from the primary containment into the secondary containment <br />in the diesel piping does not flow into a collection sump. All secondary containment systems shall be <br />sloped so that all releases will flow to a collection sump located at the low point of the underground <br />piping. The test boots were pulled back at the time of inspection. <br />307. The January 10, 2012 secondary containment test results indicate a failure at the satellite #1 piping <br />run and the dispenser #2 satellite/ 1 master. The initial and final reading results for the diesel sump <br />indicate a failure, however "pass" is marked on the results form. The initial, final, and change in reading <br />results indicate a failure for dispenser #2/4, but both "pass" and "fail" are marked on the results. Also, <br />there is no dispenser #2/4 or 2 satellite/1 master on the site. All secondary containment for the UST <br />system must be tight. Immediately have a properly licensed, trained, and certified contractor repair or <br />replace the failed components under permit and inspection of the EHD. <br />Complete and submit a copy of the Return to Compliance Certification to the EHD by April 21, 2012 <br />along with a statement documenting the corrective actions that have been or will be taken for each <br />violation and any supporting paperwork. <br />Note: The secondary containment testing was conducted on January 10, 2012.
