Kettleman Ln. Lodi, Ca 95240 Program: HW <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS II, or MINOR-Notice to Comply) <br /> On site to conduct the routine inspection. <br /> Minor-Notice to Comply: 7, 45, 49, 81, 83, 84 <br /> #7 At time of inspection I observed a broken Wynn's Tran Serve 2 flush and fill machine that had <br /> approximately 5 gallons of unknown liquid (possibly transmission fluid) inside the machine. Peter Kroeze <br /> informed me that this machine is a waste and is in the outside dumpster area until it can be disposed of. <br /> A hazardous waste determination has not yet been made regarding the fluid inside this machine. A <br /> hazardous waste generator must make a title 22 hazardous waste determination with any waste <br /> generated from this facility. Correct immediately. First make a title 22 hazardous waste determination <br /> and then dispose of properly. <br /> #45 At time of inspection I observed that the 2003 contingency plan is not current. A large hazardous <br /> waste generator must have a current contingency plan. Correct immediately. Update this facility's <br /> contingency plan and submit the corrected copy to this office by 6-27-10. <br /> #49 At time of inspection the personnel training records for the employees of this facility were not <br /> available for review. Large generators of hazardous waste must have personnel training records. <br /> Correct immediately. Submit a copy of these documents to this office by 6-27-10. <br /> #81 At time of inspection I observed a broken Wynn's Tran Serve 2 flush and fill machine stored in the <br /> outside dumpster area. Universal electronic waste must be stored in a manner that prevents breakage <br /> and release to the environment. Correct immediately. <br /> #83 At time of inspection I observed that the broken Wynn's Tran Serve 2 flush and fill machine was not <br /> labeled as universal electronic waste. Universal waste must be labeled to identify type. Correct <br /> immediately. Label as universal electronic waste. <br /> #84 At time of inspection I observed that the broken Wynn's Tran Serve 2 flush and fill machine did not <br /> have documentation to indicate how long this has been a waste. The universal waste generator must <br /> demonstrate the length of time the universal waste has been accumulated from the date it became a <br /> waste or was received. Correcty immediately. <br /> Corrected on Site: 20, 23 <br /> #20 1 observed that the used antifreeze 55 gallon drums had blank hazardous waste labels. Peter <br /> Kroeze labeled the hazardous waste drums with 1. generator name & address 2. contents 3. physical <br /> state 4. hazardous properties 5. accumulation start date. Corrected on site. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($115). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT NY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Arts Cacapit Received _ e Title: <br />
