SUMMARY OF VIOLATIONS CLASS I,CLASS 11,or MINOR-Notice to Comply) On site to conduct a routine hazardous waste inspection. Minor Notice to Comply: 109 & 117. 109. The facility contingency plan was not on site. Each owner or operator shall have a contingency plan for the facility. The contingency plan shall be designed to minimize hazards to human health or the environment from fires, explosions, or any unplanned sudden or non-sudden release of hazardous waste or hazardous waste constituents to air, soil, or surface water. Correct immediately by preparing a contingency plan for this facility. Submit a copy to the EHD by 11-19-11. 117. The personnel training records were not on site. The owner or operator shall maintain the following documents and records at the facility: (1) the job title for each position at the facility related to hazardous waste management, and the name of the employee filling each job; (2) a written job description for each position listed under subsection (d)(1) of this section. This description may be consistent in its degree of specificity with descriptions for other similar positions in the same company location, or bargaining unit, but shall include the requisite skill, education, or other qualifications, and duties of facility personnel assigned to each position; (3) a written description of the type and amount of both introductory and continuing training that will be given to each person filling a position listed under subsection (d)(1) of this section; (4) records that document that the training or job experience required under subsections (a), (b), and (c) of this section has been given to, and completed by facility personnel. Correct immediately by maintaining the required training documentation. Submit a copy of personnel training records for this facilily to the EHD by 11-19-11. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
