Lyon to contact AES to be sure that all manifests presented to Lodi are accurate and <br />complete. I spoke with Rui, the manager in charge of the drivers at AES and he stated he will <br />review this requirement with his drivers. <br />2. 1 reviewed the manifest and its proper completion with Roman Briseno, manager of Oilstop in <br />Lodi, so that he knows how the properly completed manifest should look. This review included <br />the proper EPA #, (CAL000368140) the name, address, identification #, contact person, and <br />telephone # of the generator, and the signature of the generator's representative; the date of <br />shipment; the manifest #; the volume of waste received; proper waste codes; waste stream type <br />listed in subdivision (c); proper shipping description; including hazardous class and identification <br />#; the name, address, and identification # of the authorized facility to which the hazardous <br />waste will be transported; the transporters name, address, and identification #; the drivers <br />signature; and a statement by the generator, certifying that the generator has established a <br />program to reduce volume and toxicity of the hazardous waste to the degree, as determined by <br />the generator, to be economically practical. <br />
