Alegre Trucking, Inc. <br /> 5100 W. Highway 12 <br /> Lodi, CA 95242 <br /> Dear Mr. Ruvalcaba, <br /> Please accept this letter in response to your routine inspection conducted on October 5, 2018 at <br /> our facility located at 5100 W. Highway 12, Lodi California 95242. During your inspection, <br /> seven (7) violations were observed and noted on the Aboveground Petroleum Storage Act <br /> Inspection Report (Inspection Report) which you presented to Frank C. Alegre Trucking, Inc. <br /> (Alegre Trucking) at the conclusion of the inspection. <br /> According to the requirements stated in the Overall Inspection Comments Section(Page 6 of 6), <br /> in the Inspection Report, Alegre Trucking is required to "Complete and submit a copy of the <br /> Return to Compliance Certification Form to the EHD with a statement documenting the <br /> corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork,by November 4, 2018." We missed this deadline because it took Alegre Trucking <br /> some additional time to complete the tasks outlined in the Notice of Violation(NOV). <br /> Therefore, enclosed you will find the following documents which will show that Alegre <br /> Trucking has made every effort to comply with the requirements of the San Joaquin County <br /> Environmental Health Department's (EHD) Inspection Report, dated October 5, 2018. <br /> • Cover Letter <br /> • Return to Compliance Certification Form <br /> • Alegre Trucking Corrective Actions for the 10-05-2018 Inspection <br /> • Secondary Containment Diagram <br /> Page 1 of 2 <br />
