PHS/EHD was the lead agency providing <br />the oversight of the investigation and cleanup. Due to the high concentrations of gasoline and <br />MTBE in the soil and groundwater and the close proximity of the City of Lodi Municipal Well # 12 <br />to the release, a Cleanup And Abatement Order No. 5-00-704 was written by the California <br />Regional Water Quality Control Board, Central Valley Region (CVRWQCB) and filed on April 20, <br />2000. <br />PHS/EHD and CVRWQCB met on May 10, 2000 to review the site status and file information. <br />PHS/EHD is the permitting agency for wells and soil borings in San Joaquin County. Monitoring <br />Well (MW) permits for MW -5A, MW -8B, MW -11A, and MW -11 B are approved and enclosed. A <br />permit for MW -10A and MW -10B will be issued when an access agreement is finalized between <br />Chevron and the current property owner at the proposed location.
