The designated operator failed to document all the alarms from the attached alarm history on the <br /> September 2011 designated operator monthly inspection report and failed to check that they were <br /> responded to appropriately. The missing alarm include: L6 91 Sump STP fuel alarm on September 7, <br /> 2011 at 8:14 AM. The September report stated "No followup required". Although the work order was <br /> attached, the Designated Operator still needs to document on the monthly report the alarms that had <br /> occur and its response. During the monthly inspection, the designated operator shall review the alarm <br /> history for the previous month, check that each alarm was documented and responded to appropriately, <br /> and attach a copy of the alarm history with documentation taken in response to any alarms to the <br /> monthly report.
