Secondary containment testing was last performed on 01/02/2008 and was due by 01/2011. Testing <br /> has not been completed and is over one month past due. Secondary containment testing is required <br /> once every 36 months (3 years). Immediately schedule this test and provide 48 hours notification to the <br /> EHD. <br /> 17. The 91 line leak detector failed to detect a leak when tested. All line leak detectors shall be capable <br /> of detecting a 3-gallon per hour leak at 10 psi. The leak detector was replaced and retested during the <br /> inspection. Serial # 10051164 model leak detector VMI 99 LD 2000. Obtain a permit from the EHD for <br /> this work within one business day. If a permit is not obtained within one business day, a "Failure to <br /> Obtain a Permit" penalty will apply. <br /> 23c. The designated operator failed to document the alarm from the attached alarm history on the <br /> designated operator monthly inspection report and failed to check that they were responded to <br /> appropriately. The missing alarms include: July 6, 2010 high liquid alarm for L1: 87 annular. It was <br /> documented on the employee's July monthly log. During the monthly inspection, the designated <br /> operator shall review the alarm history for the previous month, check that each alarm was documented <br /> and responded to appropriately, and attach a copy of the alarm history with documentation taken in <br /> response to any alarms to the monthly report. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the report. <br /> Submit to SJC EHD within 30 days (03/25/11): <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for the above violation <br /> 3. test notification for secondary containment/SB989 and submit results within 30 days <br /> 4. permit application for the 91 LLD within one business day <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Ins or Received 13'.
