CLEAN-UP AND ABATEMENZ ORDER NO 5-00-704 FORMER CHEVRON STATION 49-5775 _301 WEST KETTLEMAN LANE LODI S kN JOAQUIN COUNTY Site -Specific linestigation and Cleanup Tasks (continued) 3 Install and continuously operate a soil vapor extraction system, and an interim pump and treat system in the area of the highest soil and zroundwater MtBE contamination The interim pump and treat sN stem must stabilize the groundwater plume to prey ent movement of AltBE toiNard Cahn Well 12 Compliance Date 3 July 2000 4 Submit a complete Problem Assessment Report (P A -R) acceptable to the 24 July 2000 Board and County staff, as described in Appendix A of the Tri -Regional Recommendations The PAR must include requirements for any discharge permit(s) required and a time line for submittal of Report(s) of Waste Discharge 4 • 5 Within 15 days of Board staff approval of the Problem Assessment 28 August 2000 Report, but no later than 28 August 2000, submit a Fmal Remediation Plan (described in Appendix A) for the cleanup of the soil and Groundwater at the site 6 Within 30 days of Board staff approval of the selcme;d Remedial Option, 6 October 2000 but no later than 6 October 2000, implement and continuously operate, the approved remediation system, with the exception of penodic and scheduled maintenance Any interruption in the ope ation of the cleanup system(s), other than for scheduled maintenance, without prior approval of the Executive Officer, is a violation of this Order Reporting and Permits 7 Notify the Board and County staff at least two business days pnor to any site work or sampling 8 Obtain all necessary local and state permits prior to b egmmng work 9 Submit all reports and correspondence to the Board's Sacramento Office, the San Joaquin County Environmental Health Division, and all parties (or representatives) identified in this Order Under penalty of perjury, the technical repor's as directed, which shall be submitted to the Executive Officer of the Board, and to the County, must be signed by an appropriately licensed professional (technical reports and plans, including drilling logs and soil and groundwater sampling, are to be prepared, signed, and certified b1 a Registered Geologist, Registered Civil Engineer, Certified Engineenng Geologist, or Certified Hydrogeologist) All registered professionals must be licensed by the State of California 10 Submit preapprovals with workplans and all reports it ith reimbursement requests to the State Water Resources Control Board Cleanup Fund in a Tamely manner to avoid delays that would . result in non-compliance with this Order
