iMonitoring Well installation Report - Former Chevron ServlLe station #9-5775 <br />June 2 1999 <br />located just outside the ih-mile radius of the subject site The search results are summarized in Table 1 <br />and well locations are shown on Figure 3 The monitoring wells (1 and 5 on Figure 3) are located <br />approximately 500 feet west of the subject site and belong to Arco Products Company City of Lodi <br />municipal well #6R (2 on Figure 3) is located approximately 0 6 miles northeast of the subject site City <br />of Lodi municipal well #18 (3 on Figure 3) and one domestic well (4 on Figure 3) are located <br />approximately 0 6 miles south of the subject site The CDWR file did not contain information regarding <br />the City of Lodi municipal well located in the southwestern vicinity of the subject site <br />The City of Lodi Water Division (CLWD) was contacted to obtain information regarding the municipal <br />well located in the southwestern vicinity of the subject site Based on the information provided by the <br />CLWD municipal well #12 is located at 1412 South Hutchins Street, approximately 600 feet southwest <br />of the subject site (6 on Figure 3)' This well was installed in 1962 The well is 510 feet deep with a screen <br />extending from 210 to 500 feet bgs and a grout annular seal at the depth of 108 feet bgs First groundwater <br />was encountered to the well at 55 feet bgs Annual well production has ranged from 24 4 to 165 million <br />gallons Between 1987 to 1992 the well was not used Groundwater samples collected from the well in <br />1996, 1998, and first quarter 1999 did not contain MtBE The well data is included in Appendix B <br />A review of the USGS topographic map (Lodi South Quadrangle, California -San Joaquin Co , 7 5 Minute <br />Series, 1968, Photorevised 1976) indicated the presence of three wells approximately tfz-mile southeast of <br />• the subject site and one well approximately 900 feet southwest of the subject site The CDWR file did not <br />contain information regarding these wells <br />C7 <br />4.0 FIELDWORK <br />Field work was conducted in accordance with GR's Field Methods and Procedures (Appendix C) and the <br />Site Safety Pian dated January 21, 1999 A well installation and soil boi ing permit (#018004) was obtained <br />from the SJCPHS, an underground utility locator was contracted to clear boring locations, and <br />Underground Service Alert was notified prior to drilling at the site A copy of the permit and copies of <br />the State of California Well Completion Reports are included in Appendix D <br />4.1 Dril1ing Activities <br />On January 25 through 27, 1999, a GR geologist observed V&W Drilling, Inc (C57 #720904) drill five <br />on-site soil borings (SB -1 and MWA through MW4) and install groundwater monitoring wells in borings <br />MW -1 through MW -4 at the locations shown on Figure 2 Soil boring SB -1 was drilled to 72 5 feet bgs <br />and well borings MW -1 through MW4 were drilled to 65 feet bgs using 8 -inch hollow -stem augers driven <br />by a truck -mounted drill rig Soil boring SB -1 was not drilled through the existing conductor casing as <br />specified in the Work Plan, because the casing was not perfectly vertical Soil boring SB -1 was drilled <br />approximately two feet north of the conductor casing Soil samples were collected from borings SB -land <br />MW -1 through MW -3 continuously and from boring MW -4 approximately every 5 feet The GR geologist <br />prepared logs of each boring and screened the soil samples in the field for the presence of volatile organic <br />compounds Screening data are presented on the boring logs (Appendix D) <br />34640804-2 <br />
