The forms being used pre -date the <br />SPCC plan and lack critical inspection critera as defined in the SPCC plan. Inspections and tests must be conducted <br />and stored in accordance with the written procedures developed for this facility in the SPCC Plan. Immediately <br />begin conducting all inspections and tests in accordance with the procedures in the SPCC Plan, or amend the Plan <br />and have it recertified by a Professional Engineer to accurately reflect the inspection procedures currently followed <br />at the facility. <br />This is a Class II violation. <br />712 <br />CFR 112.8(c)(6) Failed to test or inspect each container for integrity based on industry standards. <br />The 20000 gallon J-24 avaition fuel tank was not tested by a qualified individual who is certified in Steel Tank <br />Institute's (STI) SP001 industry standards. The SPCC plan states that this test must be conducted at least once <br />every twenty years. Each aboveground container shall be tested and inspected for integrity on a regular schedule <br />and whenever repairs are made. The qualifications of personnel performing tests and inspections, frequency and <br />type of testing and inspections that take into account container size, configuration, and design shall be determined in <br />accordance with industry standards. Examples of these integrity tests include, but are not limited to: visual <br />inspection, hydrostatic testing, radiographic testing, ultrasonic testing, acoustic emissions testing, or other systems <br />of non-destructive testing. Comparison records and other records of inspections and tests must be maintained on <br />site. Immediately conduct the necessary testing and submit a copy of the test results to the EHD, or provide <br />equivalence as allowed by CFR 112.7(a)(2). <br />This is a Class II violation. <br />724 <br />CFR 112.8(d)(4) Failed to regularly inspect aboveground valves, piping, and appurtenances. <br />The current inspection reports did not completely cover all piping, valves or appurtenances in the inspection. All <br />aboveground valves, piping, and appurtenances shall be regularly inspected. Immediately inspect aboveground <br />valves, piping, and appurtenances and ensure inspections are done regularly, or provide equivalence as allowed by <br />CFR 112.7(a). <br />This is a Class II violation. <br />Overall Inspection Comments: <br />An inspection checklist was provided to the facility operator on the day of inspection. The report was completed <br />and amended by the EHD. The amended report replaces the initial report. <br />On site to conduct a routine aboveground storage tank inspection. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by August 27th, 2016. <br />Page 4 of 5 <br />
