Page 47 of 179 <br />May 2, 2012 <br />6� f <br />MAY 2 6 2017 <br />Chevron <br />22888 Kasson Rd, <br />Banta, CA HcALT <br />Attn: Zach Pecor <br />The following repairs were conducted on tank T-23 at the Chevron facility located in Banta, CA. <br />3/28112 <br />• Mark -out new 24" manway for sandblasting <br />3/29112 & 3130112 <br />• Install new 24" manway <br />4112/12 <br />• Install (5) square Y4" patch plates with rounded corners to cover bottom pitting at the <br />locations specified by DJA Inspection report and vacuum box patches. <br />Install telltale holes in nozzle repads D and E. (Repad at G was too small to drill hole) <br />• Modify the water draw support for seismic isolation and install 12" dia. x'/4" reinforcing <br />plate at the base of the support. <br />Nozzle D was a hot tap and failed the repad air test due to lack of welding around the perimeter <br />of the nozzle at the tank interior. <br />4/18/12 <br />• Weld the inside perimeter of nozzle D and air test the repad. <br />Thank you for the opportunity to work with you on this project. <br />Regards, <br />Austin Moreau <br />Project Manager <br />Matrix Service, Inc. <br />
