SAN JOAQUIN COUNTY— Environmental Health Department AMENDEP Aboveground Petroleum Storage Act Inspection Report Facility Name: Facility Address: Date: CHEVRON PRODUCTS COMPANY #1001621 22888 S KASSON RD, TRACY January 14, 2016 SUMMARY OF VIOLATIONS (CLASS I, CLASS II, or MINOR - Notice to Comply) Item # Remarks 711 CFR 112.8(c)(6) Failed to perform scheduled tank tests and inspections by appropriately qualified personnel. The following was noted at the time of inspection 1) No integrity testing (external or internal) documentation was available for the T-25 Tank, which was Installed in 2005. 2) Only an "out of service" Integrity testing documentation was available for the T-131 Tank, this tank was operational at the time of this inspection. 3) Action items (for the needed repairs) were identified In the integrity testing report for the T-23 Tank (dated 4-2-12) and for the T-137 Tank (dated 10-17-2012), no follow up integrity testing documentation was available for this tank, and no records, indicating that the required repairs were completed, were available for review at the time of inspection. Each aboveground container shall be tested and inspected for integrity on a regular schedule and whenever repairs are made. The qualifications of personnel performing tests and Inspections, frequency and type of testing and inspections that take into account container size, configuration, and design shall be determined In accordance with industry standards. Examples of these integrity tests include, but are not limited to: visual Inspection, hydrostatic testing, radiographic testing, ultrasonic testing, acoustic emissions testing, or other systems of non-destructive testing. Comparison records and other records of inspections and tests must be maintained on site. Immediately conduct the necessary testing and submit a copy of the test results to the EHD, or provide equivalence as allowed by CFR 112.7(a)(2). This is a Class II violation. Overall Inspection Comments: The report was amended by the EHD on 12/22/17. The amended report removes these violations: CFR 112.7(c) Failed to provide secondary containment, diversionary structures, or equipment to prevent discharge: CFR 112.8(c)(2) Diked area is not sufficiently impervious to contain discharged oil; CFR 112.8(c)(2) Failed to provide and maintain adequate secondary containment. The amended report replaces the initial report. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, within 30 days of receiving the complete inspection report. Please be aware as of January 1, 2013, all businesses are required to submit all new (or any changes to existing) Aboveground Petroleum Storage Act (APSA) information online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov in addition to any other relevant activities and required fields. No later than January 1, 2014, all APSA data must be entered and or updated in CERS. AFTER THE COMPLIANCE DATE, EHD WILL BILL FOR ALL TIME AND ACTIVITIES ASSOCIATED WITH BRINGING THIS FACILITY BACK INTO COMPLIANCE.
