EI Dorado St., Stockton Program: APSA <br /> SUMMARY OF VIOLATIONS <br /> CLASS I, CLASS II, or MINOR-Notice to Comply) <br /> 517. Copies of inspection and testing records for overfill protection systems, level indicator, and annular <br /> space testing were not found on site. According to the SPCC Plan, these inspections and tests are <br /> conducted by the facility. Inspections and tests must be conducted in accordance with the written <br /> procedures developed in the SPCC Plan. Records of these inspections and tests must be signed by the <br /> appropriate supervisor or inspector and kept on site with the SPCC Plan for a period of three years. <br /> Immediately locate a copy of all inspection and testing records for the last three years, maintain them on <br /> site, and submit copies to the EHD by June 21, 2014. REPEAT VIOLATION <br /> 518. Annual training is not being conducted for the aboveground petroleum storage program and <br /> containers. At a minimum, oil-handling personnel must be trained in the operation and maintenance of <br /> equipment to prevent discharges, discharge procedure protocols, applicable pollution control laws, rules <br /> & regulations, general facility operations, and the contents of the facility's SPCC Plan. Annual briefings <br /> must be conducted to assure adequate understanding of the SPCC Plan and highlight and describe any <br /> known discharges or failures, malfunctioning components, and any recently developed precautionary <br /> measures. Immediately conduct necessary training for all oil handling personnel. Submit copies of <br /> training records to the EHD by June 21, 2014. REPEAT VIOLATION <br /> 518. This facility does not have a designated person for discharge prevention. The SPCC Plan shall <br /> designate a person at the facility who is accountable for discharge prevention and who reports to facility <br /> management. Immediately designate a person to be accountable for discharge prevention and update <br /> the SPCC Plan to include this information. <br /> 609. Integrity testing has not been conducted on the aboveground petroleum tank. According to the <br /> SPCC Plan, the facility is using visual inspections in lieu of integrity testing. Integrity testing must be <br /> conducted in accordance with applicable industry standards. The facility shall test or inspect each <br /> aboveground container for integrity on a regular schedule and whenever you make material repairs. The <br /> facility must determine, in accordance with industry standards, the appropriate qualifications for <br /> personnel performing tests and inspections, the frequency and type of testing and inspections, which <br /> take into account container size, configuration, and design (such as containers that are: shop-built, field- <br /> erected, skid-mounted, elevated, equipped with a liner, double-walled, or partially buried). Examples of <br /> these integrity tests include, but are not limited to: visual inspection, hydrostatic testing, radiographic <br /> testing, ultrasonic testing, acoustic emissions testing, or other systems of non-destructive testing. <br /> Comparison records and other records of inspections and tests must be maintained on site with the <br /> SPCC Plan. Immediately conduct all necessary integrity testing and maintain records on site as <br /> required.
