SAN JOAQUIN COUNTY <br /> ENVIRONMENTAL HEALTH DEPARTMENT <br /> 600 East Main Street, Stockton, California 95202-3029 <br /> Telephone:(209) 468-3420 Fax: (209)468-3433 Web:www.s*gov.org/ehd/unitiii.htmi <br /> CONTINUATION FORM Page: 4 of 5 <br /> OFFICIAL INSPECTION REPORT Date: 12/17/10 <br /> Facility Address: 166 Frank West Cir., Stockton Program: APSA <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS 11,or MINOR-Notice to Comply) <br /> ROUTINE ABOVEGROUND PETROLEUM STORAGE ACT (APSA) INSPECTION <br /> Violations: (19, 21, 24, 27, 48, 50) <br /> 19, 21 & 27. The inventory listed in the SPCC Plan does not include the oil totes and only includes a <br /> maximum of 300 gallons in drum storage. Thirty-one oil totes and sixty 55 gallon drums of oil were <br /> observed at the time of inspection. Discharge & drainage controls and potential equipment failure are <br /> not addressed for any of the unlisted containers. The SPCC Plan must include the correct type of oil <br /> and storage capacity of each fixed storage container, and either the type of oil and storage capacity of <br /> each portable container or an estimate of the potential number of portable containers, types of oil, and <br /> anticipated storage capacities. The Plan must include discharge or drainage controls, such as <br /> secondary containment around all containers and other structures, equipment, and procedures for the <br /> control of a discharge, and a prediction of the direction, rate of flow, and total quantity of oil which could <br /> be discharged as a result of each type of major equipment failure. Immediately update the inventory and <br /> discharge information in the SPCC Plan to accurately represent the petroleum storage at the facility. <br /> 24. The phone number listed for San Joaquin County Office of Emergency Services is outdated. The <br /> plan must include a contact list and current phone numbers for the facility response coordinator, national <br /> response center, cleanup contractors, and all appropriate federal, state, and local agencies who must be <br /> contacted in case of a discharge. Immediately update the contact list to include the most current version <br /> of the required information. <br /> 48. Records of integrity testing is not being maintained at this facility. The 550 gallon waste oil tank is <br /> not being integrity tested. Each aboveground container must be tested or inspected for integrity on a <br /> regular schedule and whenever you make material repairs. Comparison records and other records of <br /> inspections and tests must be maintained on site. Immediately begin integrity testing in accordance with <br /> the facility's SPCC plan, and maintain records on site for three years. <br /> 50. The 550 gallon waste oil tank does not have a liquid level sensing device. High liquid level alarms <br /> must be installed in accordance with 40 CFR 112.8 and shall be regularly tested to ensure proper <br /> operation. Immediately install and begin regularly testing an approved liquid level sensing device on this <br /> tank. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Insp b: Received By: Title: <br /> EHD 202-0R 081110/110 CONTINUATION FORM <br />
