The name of the <br /> company, names and numbers of designated person(s)for discharge prevention have not been updated.The two <br /> 15,000 gallon diesel tanks appear to be manifolded and the plan does not address this. If the tanks are determined <br /> to be permanently manifolded,the tanks may be considered one tank.A 120 gallon oil tank has been removed from <br /> the facility and is still referenced in the SPCC plan. Employee training section references GTI's Emergency <br /> Response Manual for Initial Response Procedures and Countermeasures Implementation. GTI is the name of the <br /> previous business that occupied the facility.The Spill Prevention, Control, and Countermeasure(SPCC) Plan must <br /> be amended when there is a change in the facility design, construction, operation, or maintenance that materially <br /> affects its potential for a discharge,within 6 months of the change, and implemented as soon as possible, not later <br /> than 6 months following preparation of the amendment. Immediately make all necessary amendments to the SPCC <br /> Plan to accurately represent the procedures and policies currently in place at the facility. <br /> Note: <br /> Permanently manifolded tanks are tanks that are designed, installed, or operated in such a manner that the multiple <br /> containers function as a single storage unit(67 FR 47122, July 17, 2002).Accordingly, the total capacity of <br /> manifolded containers is the design capacity standard for the sized secondary containment provisions(plus <br /> freeboard in certain cases). <br /> This is a minor violation. <br /> 618 CFR 112.7(e), 112.8(c)(6) Failed to keep records of procedures, inspections,or integrity tests for three years. <br /> Copies of inspection and testing records for were not found on site. Inspections and tests must be conducted in <br /> accordance with the written procedures developed in the Spill Prevention, Control, and Countermeasure(SPCC) <br /> Plan. Records of these inspections and tests must be signed by the appropriate supervisor or inspector and kept on <br /> site with the SPCC Plan for a period of three years. Immediately conduct inspections and tests as outlined in the <br /> SPCC plan. <br /> This is a Class II violation. <br /> 619 CFR 112.7(f)(1) Failed to train personnel on all discharge prevention details listed in this section. <br /> Oil handling personnel were not trained. At a minimum, oil handling personnel shall be trained in the operation and <br /> maintenance of equipment to prevent discharges; discharge procedure protocols; applicable pollution control laws, <br /> rules, and regulations; general facility operations; and the contents of the Spill Prevention, Control, and <br /> Countermeasure Plan.
