The following is an itemized list of aboveground petroleum storage act violations that <br /> have not been addressed for VALLEY PACIFIC HWY99 CARDLOCK as of May 06, <br /> 2017. <br /> Open violations from March 20, 2017 inspection <br /> Violation #618 - Failed to conduct inspections or integrity tests or maintain records for 3 years. <br /> The 55 gallon drum located adjacent to the 12,000 gallon aboveground tank, is not being inspected. Inspections <br /> and testing shall be conducted on all aboveground liquid petroleum containers larger than 55 gallons, including all <br /> 55 gallon drums of oil. Records of these inspections and tests shall be signed by the appropriate supervisor or <br /> inspector and kept on site with the Spill Prevention, Control, and Countermeasure (SPCC) Plan for a period of three <br /> years. Immediately begin necessary testing and inspections for all Aboveground Petroleum Storage Act regulated <br /> containers and maintain on site with the SPCC Plan. <br /> Violation#710 - Plan failed to adequately discuss procedures to test or inspect each container for integrity. <br /> The SPCC plan failed to adequately discuss procedures for the tank inspection for the 12,000 gallon aboveground <br /> tank and the 55 gallon tank. <br /> Each aboveground container shall be tested and inspected for integrity on a regular schedule and whenever repairs <br /> are made. The qualifications of personnel performing tests and inspections, frequency and type of testing and <br /> inspections that take into account container size, configuration, and design shall be determined in accordance with <br /> industry standards. Examples of these integrity tests include, but are not limited to: visual inspection, hydrostatic <br /> testing, radiographic testing, ultrasonic testing, acoustic emissions testing, or other systems of non-destructive <br /> testing. Comparison records and other records of inspections and tests must be maintained on site. Immediately <br /> conduct the necessary testing and submit a copy of the test results to the EHD, or provide equivalence as allowed <br /> by CFR 112.7(a)(2). <br /> Violation #712 - Failed to test or inspect each container for integrity based on industry standards. <br /> The 12,000 gallon aboveground tank, and the 55 gallon drum were not tested. Each aboveground container shall <br /> be tested and inspected for integrity on a regular schedule and whenever repairs are made.
