Report completed on June 22, 2011. Checklist left on site at <br /> time of inspection. <br /> Violations: (18, 19, 24) <br /> 18. The facility diagram did not list the contents of each tank. The Plan shall include a facility diagram <br /> which must mark the location and contents of each fixed storage container and the storage area where <br /> mobile or portable containers are located. It must identify the location of and mark as "exempt" <br /> underground tanks. It must also include all transfer stations and connecting pipes, including intra-facility <br /> gathering lines. Immediately update the facility diagram to include all of the required information. <br /> Submit a legible copy of the updated facility diagram to the EHD by July 22, 2011. <br /> 19. The inventory listed in the SPCC Plan does includes two different shell capacities for the red dye <br /> (tank #275). The SPCC Plan must include the correct type of oil and storage capacity of each fixed <br /> storage container, and either the type of oil and storage capacity of each portable container or an <br /> estimate of the potential number of portable containers, types of oil, and anticipated storage capacities. <br /> The inventory was updated with the correct information at the time of inspection. <br /> 24. The contact list does not include the phone number for San Joaquin County Office of Emergency <br /> Services (OES) or the clean up contractor. The State Office of Emergency Services has been changed <br /> to the California Emergency Management Agency (CalEMA). The SPCC Plan must include a contact <br /> list and phone numbers for the facility response coordinator, National Response Center, cleanup <br /> contractors with whom you have an agreement for response, and all appropriate federal, state, and local <br /> agencies who must be contacted in case of a discharge. The contact list was updated with the correct <br /> information at the time of inspection. <br /> Complete and submit a copy of the Return to Compliance Certification to the EHD by July 22, 2011 <br /> along with a statement documenting the corrective actions that have been or will be taken for each <br /> violation and any supporting paperwork. <br /> Provided: Return to Compliance Certification form <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY_I_S SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> EHD 23-02-003 Re 8/10/10 CONTINUATION FORM <br />
