RECOMMENDATIONS, CLEANUP and FINAL DISPOSITION) <br />11/15/10 6:15 PM <br />Received a message from the operator that Mr. Mike Elisason with Valley Pacific Petroleum Services called to <br />report a diesel fuel spill at 3550 S. Hwy 99 Frontage Rd., Stockton. (209) 993-8793. <br />6:21 PM - I called Mr. Eliason and left a message asking for more information. <br />Mr. Eliason called me back and informed me that Elite IV Contractors was hired to make a repair to dispensing unit <br />3/4. Elite IV neglected to shut the power off to the pump and a customer lifted a handle which caused the pump to <br />come on and cause the spill. The shear valve on the pipe did not hold. The pump was shut down and the spill was <br />contained with absorbent. The service technician for Elite IV was sprayed with diesel fuel but was not harmed <br />according to Mr. Eliason. The spill did not reach a storm water catch basin. Mr. Eliason stated that they would <br />resume the clean up at 8:30 AM tomorrow. I informed him that I would be on site tomorrow to document the spill. <br />11/16/10 8:06 AM <br />On site to document the clean up of the spill. Met with Mr. James Abbott, Mr. Dan Elmer and Mr. Nathan Crum who <br />work for Valley Pacific Petroleum Services. They had personnel from the company sweeping up the absorbent that <br />was placed on the spill and containerizing it for pick up later by a licensed transporter. I took several photos of the <br />staining of the spill on the asphalt. The spill extended approximately 100 feet from the dispenser to the east. <br />Staining was also observed to the west of the dispensing area off of the concrete and onto the asphalt. The asphalt <br />has cracks and gaps and is in poor condition. The nearest storm water catch basin is to the south, approximately <br />200 feet and it flows to a pond to the south east. The dispenser that was being worked on is part of an <br />Aboveground Storage Tank system, so a permit to repair was not required to be obtained. The clean up will <br />continue throughout the day and I requested that personnel come back later in the week to check for weeps of fuel <br />coming out of the asphalt. I also requested that a copy of the waste disposal receipt for the waste absorbent be <br />submitted to me once it has been hauled away from the site. Left at 8:32 AM. <br />Filled out a Prop. 65. <br />11/22/10 - 11:15 AM <br />Received a call from Wendy Cohen with the CVRWQCB (916) 464-5817 who received my Prop. 65 notification. I <br />informed her of the spill and that it was released from a aboveground storage tank system. I called and spoke with <br />Mike Eliason and asked him for the exact number of gallons that was spilled. He stated that 75 gallons was the <br />estimate upon reviewing the inventory of the tank. Mr. Eliason also had a question about which report he was to <br />submit in seven days. I informed him to contact CALEMA to find out about the follow up reporting and emailed him <br />a link to the CALEMA spill reporting guidelines. I left a phone message with Wendy Cohen regarding the updated <br />gallons of fuel that was spilled. <br />11/23/10 <br />Received updated CALEMA spill report changing quantity released to 75 gallons. <br />Rev. 08/16/10 Page 3 of 3 ER Record <br />
