Based on my review, <br />please provide the following revisions/additional information: <br />Provide the staff certifications for: Veeder Root (Robert's certification has expired and Michael's certification expires on <br />3/18/17.); OPW, Franklin Fueling Systems, Fiber Glass Systems, and Smith -Red Thread piping. <br />Note that on page 2, titled 'Environmental Health Department San Joaquin County Application for Underground Storage <br />Tank Retrofit or Piping Repair Permit', Mike Koenig's ICC Installer's expiration date is listed as 2/13/16. 1 believe this <br />should be 7/21/18. <br />Page 3 —titled UST System Retrofit or Repair, #3 does not mention that some of the diesel double wall fiberglass piping <br />will be removed; and #4 does not list the part numbers. Include such things as: Bravo B-8000 for UDCs; Smith Piping — <br />Red Thread IIA and include diameters 3"/2"; include the Bravo F series listed model number(s); OPW 10 Series shear <br />valve; Fireflex flex pipe model numbers. <br />Page 4, beginning with number 5, has not been completed. It is my understanding that some of the diesel double wall <br />piping will be removed. Also, 5.e. states that Permitted Disposal Site is Love's Ripon, CA 1553 Colony Road. This must be <br />corrected. Number 8 states that soil samples will be collected 2' beneath each sump. This should be changed to one - <br />and -a -half feet since the collection depth cannot exceed two — four feet. The soil samples should be representative of <br />native soil. <br />Page 5, titled Sampling Protocol for Routine Piping, Dispenser Removal/Replacement: on the far left column, change <br />tank # to UDC and indicate 8 for UNL GAS and 2 for Diesel analysis for the two locations where diesel is currently being <br />dispensed. <br />Page 6, there is no date after Chris Weldon's signature.
