The permit has been <br /> approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. The mechanical line leak detector in the 87-octane submersible turbine pump sump was tested on <br /> 1/25/2017 and passed. No further testing is required at this time. <br /> 4. If concrete will be broken near the tank, a secondary containment test must be performed on the affected <br /> USTs once concrete is poured in accordance with the manufacture's guidelines. <br /> 5. Any individual performing the work of a service technician must meet the following qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, current <br /> contractor's licenses issued by the Contractors State License Board (may be issued to the <br /> installation company), or a Tank Tester's license issued to the individual by the State Water Board, <br /> and <br /> b. Possess or work under the direct and personal supervision of an individual physically present at the <br /> work site who possesses a valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> c. Possess a valid, current certificate of training from the manufacture(s) of the component(s) being <br /> tested, repaired, or serviced. <br /> 6. Individuals installing underground storage tank system components must possess or work under the <br /> direct and personal supervision of an individual physically present at the work site who possesses at of <br /> the following: <br /> a. A valid, current contractor's license issued by the Contractors State License Board, and <br /> b. A valid, current UST Installation/Retrofitting certificate issued by the International Code Council <br /> (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the component(s) being installed. <br /> 7. The tank owner or operator shall log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/and ensure all UST information is current. <br /> 1868 E.
