Immediately provide adequate secondary containment for all <br /> aboveground petroleum storage containers larger than 55 gallons. <br /> #31 Failed to conduct inspections and tests in accordance with written procedures and/or maintain <br /> signed records of inspections and tests on site for 3 years. <br /> Inspections are being conducted on the diesel, red diesel, and unleaded fuel tanks only. Testing is <br /> conducted only on the unleaded fuel tanks. Inspections and testing shall be conducted on all <br /> aboveground liquid petroleum containers larger than 55 gallons, including the fuel tanks, pit tank, <br /> emergency generators, used oil tank, baler machine, and all 55 gallon drums of oil. Records of these <br /> inspections and testing shall be signed by the appropriate supervisor or inspector and kept with the <br /> SPCC Plan for a period of three years. Immediately begin necessary testing and inspections for all <br /> APSA regulated containers and maintain on site with the SPCC Plan. <br /> #32 Failed to: (1) train personnel on discharge prevention; (2) designate a person to be accountable for <br /> discharge prevention; (3) schedule prevention discharge prevention briefings at least annually. <br /> Annual training is not being conducted for the aboveground petroleum storage program and containers. <br /> At a minimum, oil-handling personnel must be trained in the operation and maintenance of equipment to <br /> prevent discharges, discharge procedure protocols, applicable pollution control laws, rules & regulations, <br /> general facility operations, and the contents of the facility's SPCC Plan. Annual briefings must be <br /> conducted to assure adequate understanding of the SPCC Plan and highlight and describe and known <br /> discharges or failures, malfunctioning components, and any recently developed precautionary <br /> measures. Immediately conduct necessary training for all oil handling personnel. Submit copies of <br /> training records to the EHD within 30 days (8/2/10). <br /> #48 Failed to: test each aboveground container for integrity on a regular schedule or when repairs were <br /> made; inspect containers and container supports; or keep records of inspections and tests. <br /> Integrity testing has not been conducted on the aboveground containers. The facility shall test or inspect <br /> each aboveground container for integrity on a regular schedule and whenever you make material <br /> repairs. Examples of these integrity tests include, but are not limited to: visual inspection, hydrostatic <br /> testing, radiographic testing, ultrasonic testing, acoustic emissions testing, or other systems of non- <br /> destructive testing. Comparison records shall be maintained on site with the SPCC Plan.
