Annual training is not being conducted for the aboveground petroleum storage program and <br /> containers. At a minimum, oil-handling personnel must be trained in the operation and maintenance of <br /> equipment to prevent discharges, discharge procedure protocols, applicable pollution control laws, rules <br /> & regulations, general facility operations, and the contents of the facility's SPCC Plan. Annual briefings <br /> must be conducted to assure adequate understanding of the SPCC Plan and highlight and describe any <br /> known discharges or failures, malfunctioning components, and any recently developed precautionary <br /> measures. Immediately conduct necessary training for all oil handling personnel. Submit copies of <br /> training records to the EHD by October 20, 2012. <br /> 519. The SPCC Plan did not include discussion of lighting or access to oil storage areas. The Plan must <br /> include descriptions of how you secure and control access to the oil handling, processing & storage <br /> areas, secure master flow & drain valves, prevent unauthorized access to starter controls on oil pumps, <br /> secure out-of-service and loading/unloading connections of oil pipelines, and address the <br /> appropriateness of security lighting to both prevent acts of vandalism and assist in the discovery of oil <br /> discharges. Immediately update the Plan to include all of the required security information. <br /> 609. Integrity testing was conducted on September 24, 2001 as visual inspection. The Plan does nto <br /> mention future testing requirements. The facility shall test or inspect each aboveground container for <br /> integrity on a regular schedule and whenever you make material repairs in accordance with applicable <br /> industry standards. Comparison records and other records of inspections and tests must be maintained <br /> on site with the SPCC Plan. Immediately conduct all necessary integrity testing and maintain records on <br /> site as required. <br /> 611. Liquid level sensing devices are not installed and regularly tested on the fuel tanks. You must <br /> install at least one of the following devices in each container: <br /> - High liquid level alarm with audible or visual signal <br /> - High liquid level pump cutoff device set to stop flow at a predetermined content level <br /> - Direct audible or code signal communication between the container gauger and the pumping station <br /> - Fast response system, such as digital computer, telepulse, or direct vision gauge. If a direct vision <br /> gauge is being used for determining the liquid level of each tank, a person must be present to monitor <br /> gauges and the overall filling of the tanks. <br /> Immediately install an approved liquid level sensing device in accordance with 40 CFR 112.8 and <br /> implement necessary procedures to ensure that the devices are fully functional and in use at all times <br /> during tank filling operations. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> Stacy Rivera (209) 468-3440 <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
