December 14, 1998 IMPORTANT NOTICE Cris Hamilton Carl W & Muriel E Yearing 1062 Rodeo Rd Pebble Beach, CA 93953 NOTICE TO DISENCUMBER FUNDS FROM UNDERGROUND STORAGE TANK CLEANUP FUND CLAIM NUMBER 8790 AT SITE ADDRESS: 521 CHEROKEE ST N LODI Our letter dated August 29, 1997 requested that you submit a reimbursement request with the supporting documentation or submit a written explanation as to the status of the cleanup and when a reimbursement request could be expected. To date, we have not received a response to our letter. Your last payment request was processed on July 16, 1996. The Fund plans to disencumber any remaining fiends in the Letter of Commitment. Please notify me within thirty (30) calendar days from the date of this letter if there is any reason why the funds should not be withdrawn. If no reimbursement request is received by January 14, 1999,we will disencumber all funds and inactivate your claim. Please be reminded of the following requirements: I. Non-Recovery From Other Sources Disclosure Certification: Your claim to the Fund included the completion of a"Non-Recovery From Other Sources Disclosure Certification" farm, which you, under penalty of perjury, identified any money you may have received from other sources related to or paid in consideration of the unauthorized release. This certification also declared that if funds were received after completion of the certification, you must notify the Fund promptly and remit to the Fund the amount determined by the Fund to be double payment. Failure to comply with this requirement will result in actions bv the Fund to void the claim and demand re a ment of all funds paid In order to identify any money received since you completed the prior certification, please complete and send in the enclosed form. It is not necessary to resubmit previously submitted documentation, but you should reference that submittal. ?. Reimbursement of local oversight costs: In many cases, local regulatory agencies charge responsible parties for oversight costs. As oversight costs are eligible for reimbursement from the Fund, it is your responsibility to ensure that all such costs have been billed and reimbursed.
