Jl <br /> f 304 North Acacia { <br /> Page 3 <br /> .i <br /> Contaminated Site Reporting <br /> On March 21, 1989 an unauthorized release report and a notification of hazardous waste <br /> discharge report were filed as a result of a failed precision test. <br /> i� <br /> JI <br /> On October 23, 1991, an unauthorized release report and�!a notification of hazardous waste <br /> j discharge report were filed following observations at the time of tank piping removal. <br /> On December 6, 1991, a notification of hazardous waste discharge report was filed <br /> following observations of hydrocarbon odor and soil discoloration in the southern end of <br /> the excavation. The excavation was subsequently lined v'vith plastic and backfilled with <br /> the contaminated soil. <br /> I� <br /> PHS/EHD correspondence dated December 28, 1992, indicated that further investigation of <br /> the soil and/or groundwater contamination were required. <br /> Site Status <br /> The 10,000 gallon diesel tank #39-2243-01,continues toi:operate in compliance with <br /> regulations. <br /> The site has been placed into the local oversight program�to oversee the investigation <br /> and/or mitigation of contamination which resulted from the operation of underground <br /> I4 storage tanks. A work plan to initiate the investigation should be submitted by December <br /> 2, 1996. <br /> Enclosed you will find an application to the Cleanup Fund's In order to assist the ' <br /> completion of the application, PHSIEHD has enclosed copies of the unauthorized release <br /> reports and permits to remove the former underground storage tanks. <br />' PHSIEHD endeavors to work cooperatively with tank owners, and hopes that the extent of <br /> soil and/or groundwater contamination is limited so that aispeedy and inexpensive site <br /> certification can be obtained.
