Ad Art Road <br /> Stockton, CA 95215 <br /> San Joaquin County Public Health Services, Environm6'ntal Health Division (PHS/EHD) <br />` has reviewed the analytical results of the soil and/or water samples collected beneath <br /> the underground storage tank(s) that was/were permanently closed at this site on <br /> September 25, 1998. <br /> Based on the levels of contaminants present in the samples collected, further <br /> investigation to determine the extent of the soil and/or groundwater contamination is <br /> required. <br /> An "Underground Storage Tank Unauthorized Release';(Leak)IContaminated Site <br /> Report" has been filed pursuant to Section 25295 of the Health and Safety Code (HSC). <br /> The appropriate disclosure of a hazardous waste discharge pursuant to Section <br /> 25180.7 of the HSC has been made, and the site has been placed on the list of <br /> contaminated sites in San Joaquin County which is maintained by PHSIEHD. <br /> 'i <br /> PHSIEHD under contract with the State of California Water Resources Control Board <br /> (SWRCB) oversees the investigation and remediation of contamination from leaking <br /> UST's. Responsible parties must take corrective action in response to an unauthorized <br /> release, in compliance with Section 25299.37 of the HSC and Corrective Action <br /> Regulations adopted pursuant to Section 25299.77 of the HSC. <br /> PHSIEHD uses the SWRCB "Prioritization System for Leaking Underground Storage <br /> k Tank Sites" to prioritize all the contaminated sites.
