(AGE) <br />was conditionally approved for implementation. The deadline to have the work completed <br />was February 13, 2009, but it has not been done. <br />Please proceed with submittal of an EHD Well Permit Application, required $122.00 permit <br />fee and $366.00 permit inspection fee, an EHD Master File Record Form, and with <br />scheduling the approved work. An access agreement or encroachment permit must be <br />obtained for offsite work; please submit a copy with the EHD permit. A report of the work is <br />due for submittal to the EHD within 60 days following completion of the fieldwork. <br />This is the last deadline extension notification you will be receiving. If the permit and fees are <br />not submitted to this agency within 14 days from the date of this letter, and if the approved <br />work is not completed by December 30, 2010, this site may be referred to the Regional <br />Water Quality Control Board, Central Valley Region (CVRWQCB) for enforcement action. <br />This site is further out of compliance with regulatory directives for failing to conduct routine <br />groundwater monitoring and sampling.
